Roughly 2007 ish, in the days of Netflix DVDs sent to your door, I was at my buddies' apartment and we were watching movies I'd rented until the wee hours of the morning. (all of us were stone cold sober, I wasn't even 21) The guy that rented the apartment was sitting there watching one other guy get up to switch discs, said watch this, and then asked the guy "Uh, shouldn't you rewind that before you take it out?" And without thinking he was like oh yeah, and put the disc back in the drive...then stopped, realizing what had happened 🤣

I worked at a full serve station (remember those) and one night there was a car on the lift. Shouldn't have been because the lift was being replaced. I walked by it and two minutes late the thing slammed into the ground. I was freaking out because I was just there. My boss told me to get over it and be a man. I quit there and then.

They would SO not get away with this in Europe, where you have to display your car's latest MOT or equivalent (colour coded) in the window - they'd get a whopping fine, vehicle not allowed on the road and probably an inspection
Lots of countries in Europe do not have any such display, and in any case tyres can wear out much more frequently than even an annual test, so it's going to be down to being pulled over by police in any case. In CH where I live the test is only once every three years (or more, depending on how busy they are) but is insanely strict. Failure to attend for a test when required means the number will be flagged and police will be round to physically remove the plates, as well as automatically issuing fines.

It's Probably Fine
This isn't AS bad as it looks if they are being safe about the electricity part. Judging by the bale and the tractor this is a short pole and they are only about 15 feet above the ground. We had a couple of similar poles on our farm. That bale is on a bale spike so isn't going to tip over. It wouldn't meet OSHA standards (if in the USA) but they are probably just fixing stuff on their own farm. Hopefully the power is shut off.
EDIT: OSHA applies to all farms, but yes, congress has an exemption in place that prevents OSHA from inspecting / enforcing for farms with fewer than 10 employees. Also, it's child labor laws that now prevent kids from doing a lot of what I did back in the early 70s. Google it. Where there is more leeway is if they are just fixing stuff on their own farm and are not employees. This is a spot or mid pole and based on the tube to the genius’ right it’s probably delivering fiber or twisted pair (telephone). We throw ladders on these spans in spots our trucks can’t reach all the time just not from in our bucket. I’d be fired on the spot for anything this dumb.

If it's just oxygen then it's probably only dangerous to the person smoking (things absolutely burst into flames in oxygen-rich environments, even things that aren't normally terribly cumbustible like nasal canulas). However, if there are any other gasses stored in that room, I'd be terrified. We store all of our indoor compressed gasses in individual safety cabinets, though our tanks are much smaller. Also, posters are absolutely correct about these things being dangerous when beached. Usually you see this with the stem getting knocked off when the cylinder is knocked over, which doesn't look like an issue here.

Oxygen is not flammable, but it can cause other materials that burn to ignite more easily and to burn far more rapidly. The result is that a fire involving oxygen can appear explosive-like. Thanks Google. Point being, still dangerous, especially with smoking involved.

Somehow, miraculously, surprisingly it didn’t lead to him losing his toe and ending his career
I had a co-worker who came in one day, he had sliced his finger open really bad with a chef's knife over the weekend. He didn't have great insurance, so he used super glue. I was sure he was down lose the finger, the whole finger turned black. But he did manage to get through it. I'm so glad I don't work at that company with bad health insurance anymore.

If you have every used those little white zip ties - it is hard to see how it would stop anyone. It might confuse someone just pressing on the bar - maybe thinking the door is somehow locked, but it would never stop someone shouldering into the door to escape a fire. It's still a safety violation but more on a technicality than a real danger.
In an fire people don't think rationally, someone pressing the bar may think its locked then try to find another exit, putting them in danger.

Tape, Concrete... Same Thing
This actually works short term to prevent spalling (pieces breaking off) until the pole can be brought down safely with a crane. They are reenforced concrete, so won't fall over without taking more damage then that. You have to basically pulverize enough concrete that you expose enough rebar for it to bend

A Truck That Barely Strapped On It's Cargo
Load of different sized roof trusses, at a glance I see at least 6 three inch straps (possibly more if i zoom in). Also loaded to be balanced on the truck. Have had loads delivered looking worse than this. Delivered flat you will always have peaks and eaves hanging out everywhere. Truss trailer is the best way to haul but that is a specialized trailer.
Saw a guy at a Flying J one night, just standing behind his flatbed and staring at the load. It was all flat panels of some kind and the top had shifted over about 5 feet at the back. Well over the edge of the trailer. Two of his straps at the back had snapped, now he had a definite lean on the trailer.

People like this is why I got out of safety. These are egregious examples but I had a field engineer (a week after I safety audited him), take off his leather and rubber gloves when installing new batteries on a UPS (Uninterruptible Power Supply System.) He connected them wrong and blew off 3 of his fingers. The PPE would have made this only a minor incident. And people don't like being told what to do. Biggest lesson I learned.
Yeah, it's a very frustrating field to work in. You're simultaneously always the bad guy with the workers and with corporate. Meanwhile you're trying to keep one group alive and the other from getting their a**e sued off (or where I come from, out of jail).

So, something like half of them turned out to NOT be actual osha violations, a couple were staged for dumb reasons, and maybe one or two were actual problems. For example the stick holding up the telephone line, was later noted to be a stop gap situation used to keep a city road open while linemen were stringing a new telephone line. It was only like that for ten or fifteen minutes. Others, like the truck carrying trusses, was noted to have far more straps holding it down than it seemed at first glance, while yet others, like the zip tied fire door, were noted to not be violations, as the weight of a person hitting the door at full speed would simply break the zip ties. once again, Bored Panda trolls reddit to repost, and makes everyone think it's new, and unusual content.
Zip tie still a violation. Not allowed to obstruct or hinder in any way. Small women, children, and disabled people exist and need to be able to exit in an emergency. All your statement proves is that it wouldn't serve the purpose for which it was added.

Last I heard they'd racked up several million as they covered up for them (top 4 biggest gossip.fuel companies pretending it's about family) For the record these trucks cost $12million and when loaded weigh 780tonnes.
