This Group Is All About People Ignoring Safety As If They’re Immortal, Here Are 40 Of The Worst (New Pics)
Depending on what your job is, workplace safety can range from getting a stiff wrist to all sorts of daily life and death situations. Fortunately, at least if you are in the US, there is the Occupational Safety and Health Administration (OSHA) to make sure your employer does their utmost to prevent any injury. However, just because there is a rule doesn’t mean all employers actually follow it.
The “r/OSHA” online is dedicated to documenting the most atrocious, random and grimly hilarious workplace safety violations. So get comfortable as you scroll through, upvote your favorites and be sure to share your thoughts and experiences in the comments section below.

The More You Look The Worse It Gets
These guys must have no fear. (Sorry, I think I miss-spelled that. It's b-r-a-i-n, isn't it?)
Physics. I'd prefer a higher coefficient of friction, but I've seen far worse than this.

Before I saw, this comment, I said out loud..."No. No it is not." Well played random Stanger. Well played.
Load More Replies...Usually on these rides, the walkway and the wheel are separate - the wheel is typically on a trailer. This is definitely supporting the stairs and most likely has nothing to do with the wheel. The ground is uneven and the second photo looks to be at an angle anyway. I doubt that these supports have anything to do with the wheel itself.
Can confirm. The wheel lays on the trailer stabilizers. The walkway is just a side of the trailer that opens to the side and requires some support to avoid excessive load on the hinges. This is shaggy installation that would not pass inspection around here, but not an actual safety hazard to the wheel.
Load More Replies...It's obviously dangerous, but it's also obviously just holding up the walking platform to take people to the ride itself. However: if those in charge of the ride are happy doing something dodgy like that, I'd not trust them to ensure the ride was properly set up. Run away!
Not safe, but photo may be misleading. I am betting that the ferris wheel has a separate support structure. This is just a passenger platform waiting to fall through instead of a ferris wheel about to fall over.
Is the second photo a purposefully bad camera angle, or are people really waiting in line to ride a death trap poorly disguised as a ferris wheel?
There does seem to be a skewy angle in the second photo.
Load More Replies...I think I've seen this done at pop up fairs. I'm going to have to pay more attention now.
Safety Standards In 1960
If you ask some of us older folks we'll tell you many a tale of the "old days" when this sort of thing was the norm.
The view behind it makes it look higher than it actually is. In reality, they are only about 2 meters off the ground.
Load More Replies...The picture is deceptive. It’s been on a zillion websites in the last 30 years. It’s really not that far off the ground, but the angle of the photograph makes it look like it is.
https://www.boredpanda.com/lost-in-history-twitter/ "Photo taken in Snow King Resort, Jackson, WY. Operational between 1951 and 1978, it replaced a similar single seater arrangement operational since 1946, that reused posts and cables from an ore tram built in the '30s for a mine close by. It was replaced in 1981 be a modern Doppelmayr Garaventa double chair lift. Removed in 2014. There are several photos like this floating around because this was taken in a spot that was specifically designed for touristic photos, to give impression of being at height, while in reality the seat was barely 3 meters from the ground for the whole run. The uncropped photos show the bushes/small trees just below. It was not exactly safe, but forced perspective is the main factor here."
Load More Replies...This is a perfect example of where we should be using billionaires to test safety.
When I was in middle school, we had a telephone pole that leaned over a ditch filled with gravel. They put a rope on it and that was our play ground. The object was not to fall 6 feet into the gravel when you swung out. Kind of like the 80's version of Survivor. I also survived the game that should not be named - dodge jarts. Gen-x was given a gauntlet and those of us who made it out the other side, well, let's just say it explains why we're a little shell shocked and checked out. It's why we've been left out of the generational wars, neither side knows how we'll react if targeted, lol. We're a wildcard. Disaster-g...a34394.jpg
Oh hell no. Just a strong enough wind is all it would take to slide off.
I've ridden on similar. Usually over snow for skiing so if you fall the snow will mostly break your fall. (less so over groomed areas). I remember riding on something like this one time over not snow - like off season as these folks are doing but I can't remember where. It was many years ago.
While most countries have their own equivalent, OSHA (The Occupational Safety and Health Administration) has become the catch-all method of discussing workplace safety or lack thereof. Founded in 1970 by Richard Nixon, its task was by no means simple.
The agency was designed to “"assure safe and healthy working conditions for working men and women by setting and enforcing standards and by providing training, outreach, education, and assistance." After all, in many jobs, bad days happen, but there are “best practices” that can be put in place to limit just how often workers end up falling off ladders.

It's her having a lit cigarette while working near spilt gasoline that's the problem.
It's actually pretty hard to light gas with a cigarette...
Load More Replies...They are saying it because she's smoking not because of the cat litter.
Load More Replies...It is not that easy to catch gas on fire unless there's a ton of fumes u can throw a little cigarette in a puddle of gas and it will go out. Mythbusters did an episode on this and it was myth busted. It obv could happen under exact right circumstances but that's a slim chance
Kitty litter - ok.. safety crocs - not ok... lit cigarette - not ok. And the look on her face to boot.
A lit cigarette, even while puffing on it, cannot ignite gasoline vapors. Dozens of studies have proven that under all conceivable conditions. There is no oxygen at the combustion region of a lit cigarette and, in fact, that region is flooded with carbon dioxide. If you know anything about combustion, you'll realize that that creates an impossibility for ignition, as has been proven.
Nothing in this picture is dangerous. Gasoline vapor is heavier than air, but even if there was a vapor concentration that can be ignited and it's right at the end of the cigarette nothing's going to happen because a cigarette won't ignite gasoline vapor. Related fun fact: if you want to ignite gasoline while not actually being there you can use a cigarette with a match stuck in the non-burning end. When the cigarette burns down to the match it will ignite the match, and the match will ignite the gasoline vapor.
We All Like A Little Bit Of Shade Right?
I'm sure somebody slapped it and said, "That ain't going nowhere." and they were almost certainly right.
I am guessing that they had no hydraulic fluid leaks because that could wreck their day.
If you are surprised by the late date of OSHA’s founding, it’s worth pointing out that the US already had some job safety regulations and oversight since the 1930s, put in place by the Bureau of Labor Standards of the Department of Labor. Even this seems shockingly “late” given the prevalence of heavy industry in the US, but it’s better than nothing.
The Lock Is To Prevent The Fire Escaping

However, OSHA isn’t some all-knowing and all-seeing organization, it really does need people to report violations. As these images show, many job sites are unsafe because managers are lazy or want to cut costs. While the worst that might happen to some of us is a sore wrist or bad-posture-induced pain, in certain jobs something going wrong can lead to significantly worse outcomes.

This Is How My Great Grandpa Stored His Dynamite, Passed Away, And Left Me To Deal With It Christmas Eve
So a bit of a fun fact. There are two types of dynamite. "Wet stick" and "candlestick". Candlestick absolutely requires an electric shock via a blasting cap to set it off, its stable enough you could juggle it. Wet stick is essentially nitroglycerin infused sawdust packed into a tube. That's the truly dangerous stuff. My family comes from coal miners, and my great grandfather kept wet stick dynamite in the house. This is because he was in Pennsylvania during the coal miners battles against corporate fat cats. Back then you had to buy your supplies from the company store, and you got paid based on how much coal you brought out for the day. Didn't bring enough? Well then you just paid the company to work for them. So the coal miners were fighting back, but naturally the company owned the police and bully boys, who would be sent to violently put down any miner strikes. So my great grandfather kept the wet stick in his house in case any of those thugs came after him.
This kind of history should be taught in schools, but then it might make the workers wise up, so...
Load More Replies...I live in a mountainous area where they did a lot of blasting while building the railroad a century or so ago. Occasionally, someone will find a stockpile of old dynamite in the forest, and the government (I think the Forest Service, but it might be the military, I'm not sure) will go and explode it to get rid of it. A few years ago, there was a bunch found not far from the Interstate, and they had to close down the Interstate for a couple of hours while they took care of it.
Stored his dynamite? Is this a thing? Do lots of people have bags of dynamite in their homes? I am endlessly amazed at how my fellow humans are spending their lives.
Chained And Locked Fire Exit
It's Texas answer to gun control safety in schools. And no, I'm not saying this to be funny.
Look up the Triangle Shirtwaist Fire. Hundreds died that day because of this exact thing.
Once again, a building code violation. Somebody needs to Code Enforcement.
At my college we had a tradition called "Senior Night" where the night before the last day of class, the students would barricade themselves in the academic buildings to keep the profs out, so they couldn't hold class. We used to do things like this to the doors. BUT! The rule was that you had to keep at least ONE exit from the building clear in case of fire, and it was a firm rule that the faculty were not allowed to try to enter through that one open door. Probably still not very safe, but apparently it made it legal!
Bad if building is open. Not a big deal if closed. That's why at a lot of businesses you will see doors marked, "This door to remain unlocked during business hours". When the box store (or wherever) has a bunch of people inside the fire exits are required. When they are closed they are not.
This is why we read horrible stories about people trying and failing to escape fires. I hope the fire marshal was notified

Load More Replies...Like in Apollo 13 where Swigert (I think) puts "NO!" over the button that separates the command module from the service module so he doesn't press it while the others in still in there.
There are so many different "WTF's?!" in that simple little note, that I do not even know where to begin.
+In the Navy on my first ship, I had some maintenance to do on top of a rotating radar director. I tagged the power on switch and the brake switch at the control panel, and for good measure tagged the breaker at the power supply. One of my co-workers came in, took my tags off, and lit the thing off. This required him to take off or ignore my three tags, violate the rule which said that before moving the director he was supposed to do an on-deck inspection of the thing to make sure nothing was impeding the movement. I was up on top, felt the vibration of the motors coming on, and got quickly down, getting clear before thedirector spun rapidly around. I set the brake locally by throwing an emergency switch, and went down to see who had just tried to kill me. Even though he was a higher rank than me, I placed him on report and he had to tell the Captain why he was so stupid as to break so many rules so quickly. The Captain fined him half his pay for three months.
Also, 45 days extra duty, 2 hours each evening 6 days a week, and 45 days restriction, which meant he could not leave the ship, and had to muster five times a day with the Master-at-Arms. Also, he was reduced one grade in pay grade, but this was suspended for one year. If he didn't go to see the Captain again in that year he'd keep his grade. He and I did not speak much after that, and my supervisors did not insist that I work with him.

Only A Matter Of Time Until Maximizing Profits. Bankrupts The Whole Company
100s of racks damaged way beyond being safe. It’s a game of Jenga stacking 1000s of pounds up in overstock. Just a matter of time until something horrible happens.
Don't stack anything on it. Leave it all in the yard, take everything down. Let the upper mgmt and CEO deal with it. It's not worth the risk.
We had that kind of stuff in a printing plant in the early 00's, the guy who handled the stock was just awful on the hilo. Everything he could run into was bent from him hitting it. Rack systems like those in the pic, support columns, that guy ran into everything that didn't run from him.
I saw a stand-alone, 30 foot rack full of driveway sealer and concrete mix collapse one night. One tap from a forklift. Many bad decisions
I used to design HPS high pallet stacks like this. It's the warehouses that the worst fork lift drivers that have this type of damage.
Also managers who say "sure, that rack will hold it"
Load More Replies...O'Reilly Auto Parts store I worked at. Someone hit the shelves with a pallet jack, dislocated a shelf support by a couple inches. We did try to push it back, not happening. So, we just left it. Afterall, the shelves are also secured to the wall, right? Yes, they were, with drywall screws. No warning, when it went, it went. One loud pop and it went as far as it could go. sandwiched 2 other things against another shelf which was against an end wall. They still didn't fix it, just kept using it in its skewed position. And that was actually the least of that buildings issues.
From A Power Washer Advertisement: Look How Easy It Cuts Through Soft Material!
A good demonstrated as to why no limbs should go near the spray of a pressure washer. The minimum you would get away with is "degloved".
That's true for industrial washers that go up to 1500 bar. Pressure washers for home use are about 120 bar and not as dangerous. You can accidentally spray on your feet and barely feel it. As long as you don't put the nozzle in contact with the skin, it's safe. It takes about 180 bar to bruise skin at 15 cm distance.

Totally Safe, It's A Load-Bearing Dumpster
On some of these I would like to see the scene being set. Just how DO they get this in place?
From a safety standpoint the only problems with this is the lack of railings and the trip hazards (it looks like the bottom side of the platform is flat, so why?). Anyone who thinks that dumpster isn't rock solid doesn't understand math or physics.
It's wedged in there by its weight alone, so long as the dumpster itself doesn't deform.

Safer than some photos of construction sites. Even if it slips, it's not going far.
Well, it looks sound. Nothing is going to move that except an above lift device, probably a big digger with a nylon belt tied around it.
Just remember, every OSHA Regulation, has someone's name secretly attached to it.
New Ceiling Lights In My Workplace Are Powered By Putting 220v Mains Voltage Into Metal Railing With Scissor Clips. The Ceilings Are Low Enough That I Could Touch The Railings If I Stretch Enough. They Were Done This Way For "Aesthetics"
Might be a transformer on the ceiling? So then only 12 volts perhaps? edit: had something similar before, 12 volts, but directly connected to the transformer "ball".
Load More Replies...Are you sure that's 220? It's a very common design where you'll have a transformer supply 12/24 V to a pair of rails/steel cables etc. and you'll hang lights on those. That's a very common decorative design. And aside from being able to be shorted quite easily they do no pose any electrocution risk.
I have some in my office, however the metal is coated in plastic that has to be pierced by the lighting pins. 12v would probably be fine, 24v would defo be felt if you held that wire.
Load More Replies...I've seen lighting like that before but it's always low voltage. 24V max. never enough to hurt you.

Saw This Outside My Office Today, What A Lovely Place To Relax Before Starting The Job
This isn't so bad. I'm sure the unit is off. Worker appears to just be in the chute, not the chopping part. One time I had to crawl down into the cutter/ compacter part of a hay baler to tighten a nut and that made me feel weird. Tractor was off and I totally trusted my step dad but it still felt weird because it wasn't a place I could get out of quickly.
This is called a confined-space as there is only one entrance/exit, no room to maneuver, and contains hazards(in this case blades/press) at most jobs you need a form(confined space permit) filled out and dated before entering. That way if something goes wrong(you get hurt/stuck/pass out) the rescue workers know what to do. There have been instances where they send someone in to try and help the first person just for the person being sent in to also need rescuing. So now anytime it happens rescue workers(usually firemen) are required to retrieve the person.

Invisible Forklift
Given that that's a load bearing post, I'm going to say they're all pretty well forked!
Well, removing it before being ready to fix or replace the tube section would have been worse. S**t happens, and at least they did the right thing by leaving the fork there, thus preventing imposing excessive lateral forces on the compromised post, and creating an empty section of the profile which would have been prone to buckling.
Well, why should an entire forklift be out of commission just because there's a problem with one blade?
“How fast was he going?” came out of me spontaneously.

My daughter is a safety engineer, and she regularly has to lay down the law to staff, and occasionally to the management. She utterly refuses to sign off on unsafe practises. I’m Hoping The Oil From The Fryer Is At Least Cold When They Try This…
I hope the employees walk out when they see this. But I also wonder what else is "improvised".
This is weak! They need to remove at least half of these to make it a real challenge!
Have you ever heard of a thing called BOOTS? they work real well if you have to walk in water puddles.
Youtuber Testing A Unsecured Ceiling Fan With Sharpened Blades While Standing Underneath It And Using His Hands
Doesn't quite look like his hands are the ones going to "hit the fan".... I'd be more concerned for his eyes when the plastic hand is being spread all over the room in high speed. Shouldn't he be wearing some sort of safety glasses???? 🤔
No kidding here. I saw this and thought of a coworker from the 90's. He was standing on top of a pallet of oil, singing What's Up (4 Non Blondes) and raised his hand into a metal blade ceiling fan. Now, I'm listening to iheart as I type this post and what comes on? Yep, What's Up. Coworker is okay, just needed a few stiches.
This would be TylerTube, he is not the brightest star in the sky but he is somewhat funny on occasion. But man do i watch him on occasion and cringe quite a bit.

Great Grandpa Electrical Work In The 60s
I used to repair amplifiers. People would bring blown up amps in with the blown fuse wrapped in aluminum foil. Folks, there is usually a reason the fuse blows open! That reason is excessive current draw. Usually caused by a shorted component.

When my granny was in hospital, I saw a maintenance idiot drop an oxygen cylinder. The thing fell on the valve, and it was like a missile. It crossed all the corridor at top speed, perhaps 40 metres, and crashed into a brick wall, where it made a big hole as if the wall had been cardboard. Just as well it didn't catch anybody!
Because he dropped a cylinder he is a "maintenance idiot"?
Load More Replies...Everything is always "fine"... up til that moment it suddenly isn't anymore.
Unfortunately, many people think that way. A neighbour in my building keeps leaving the metal safety door in the stairs open because he wants light and air... His theory is "I've been living here for ten years and we've never had any problem". (I don't know what those metal doors are called. They separate the stairs from the halls in every stage of the building to prevent the fire from spreading quickly from one floor to another). I called the insurance company to let them know. Now he is angry because he got a warning, but I couldn't care less.

We have one like this. It's a disconnect for a large RTU. The breaker turns off just fine, but it takes many tries to get it to close again. Our note is to shut down the whole panel instead.

Somebody fell down the stairs while removing a pallet from the platform so they decided to install a slide..
That's a new one on me. I'm trying to figure out why they had a pallet up there anyway.
Pallet slide is to steep as the stairs are to steep. Also why would pallets be going up those stairs? The stairway and catwalk are to narrow for that, anything that can be done up there can be piped/ran back to the floor.
Is This Bad
Lack of shoring and shielding alone would be enough to shut the site down with a nice fine on the side. Allowing unrestricted passage below unsecured heavy machinery would make the inspectors' cry from the sheer volume of paperwork to be done.
Load More Replies...The "Stairs" Im Expected To Climb Daily
Nope, it's just to allow climbing with wheelbarrow. This is a construction site, and quite frankly this arrangement is not even that bad: parapets are in place and almost at the correct height, antislip bars are fairly spaced, ramp inclination is a bit too steep but little you can do about it, it's only in the necessary part without hampering the rest of the stairway. It will probably not be up to code, but it will likely pass an inspection with just an unenthusiastic verbal warning.

Well this is the factory tonight. Dumb award goes to this stupid bastard for being on his phone when the press was running. It's 9:30. And they will be here till tomorrow. If you are wondering that is high temp gasket sealer. Not gonna be easy.
The number one violation here is for the fellow on the left's fashion sense when it comes to his choice of under pantaloons.
Looks like a blowout. Thre press is a pump that was extracting the sealer, under pressure to force it through a smaller line. Someone was on their phone not paying attention when they should have been, something went wrong, and because the operator wasn't paying attention, the sealer blew out of the press under the pressure, and so went everywhere.

You could not get a trench box in there with the upper vertical pipe that's in there. They are replacing a shut off valve. It is done this way all the time and perfectly legal.

Despite the steps, the sides caved in, one of my friends broke his leg and the ambulance had difficulty getting to the location because we were far out in a farmer's field and the ground was too soggy for the ambulance to drive. It took 6 sturdy men get to get my friend out of the trench and far too much time to reach the ambulance.

I Wish This Was Fake, Saw This At A Guy's House While Doing A Marketplace Deal
I remember having this conversation with a former coworker: Me - "Hey, are you going to wear a face shield, to cut that metal plate? Because, these cutting wheels tend to break sometimes." Him - Looking at me, debating, but hesitantly wearing the face shield. 30 seconds into him cutting the plate... OH SHT... are you ok? Ohh wow, that shield saved your face!!! Basically, cutting wheel broke, a piece flew off, and broke a hole right through the shield. Luckily, it stopped it enough, that he was just nicked, and his face wasn't obliterated! Lesson - YOUR BRAIN, IS YOUR BEST PPE... Use it wisely!!
It looks like a lot of people in these examples don't have a brain in the first place. 🤷🏻

Looks highly practical to me. I've had utility knife blades snap in use before - at least there's no risk of that. A good solid handle. Likely to be plenty sharp enough for tape but not likely to be sharp enough to slice skin easily. I truly don't see the issue.
I agree with you, but in the picture it looks like he has a razor blade attached to it? When I ran stock, I just used whatever knife I carried at the time. Half the time the store wouldn't give you new blades for the case cutters. I have a 3 inch scar on my arm to prove it. The blade that made it? Looked like a saw blade, it was so bad.
Load More Replies...If that's any more dangerous than the millions of utility knives with a fixed blade it's only because the blade isn't as sharp.
One place I worked had safety cutters. The idea is if it slips the blade will retrack because it's the pressure of pulling that keeps it out. That's the theory anyway. Manager was cutting a box, knife slipped but kept cutting because it was still in the cardboard. It didn't retract until after it went art way through his thumbnail and thumb. Those things really were a POS.
That's safer than a Stanley knife in this instance.

OSHA can be kind of a joke itself if you’ve ever tried turning in your own company. I never got finished with the paperwork, but my former retired boss turned some in and he didn’t even tell me until almost a year later. Everyone else was too scared and everyone assumed it would be me. So yeah, guess who got fired eventually. At least they did fix a couple of the problems. But unless someone loses an arm or leg OSHA doesn’t seem to care.

I used to work at a steel stockholder, we would cut beams to size using an oxyacetylene torch. We had a new cutter join us who was a little lax with his connection checking. We were under strict instructions not to enter the cutting area whilst he was cutting beams. His acetylene connection was leaking at the top of the regulator, he didn’t know, he carried on cutting and the leak caught fire. He flipped out at me when I disturbed him mid cut and started to rant about safety. I got to walk away saying ‘well I won’t tell you that your bottle is on fire then….’. Fire got extinguished but the fire service attended and we got shut down until the bottle cooled down. Our proximity to Manchester Airport meant flights got a small reroute to avoid our building. Luckily no harm came to anyone or the building but our cutter got a royal rollocking. Got a plasma cutter not long after that incident 😀
The only violation I've seen at my job is the time our snow removal company found a great place to pile a post-blizzard mess...right up against our emergency exit. Nobody noticed until--get this--a fire engine was stopped at a red light and happened to see it. Fire marshal showed up and shut us down until we got the snow taken care of. Boss asked if employees could still work. Marshal gave her a great WTF look and said um...NO.
