There are many places that have urinals within eyesight of the hand washing station. And perhaps man #2 walked into the bathroom, saw the woman, and walked back outside to wait.

USUALLY, gender-neutral bathrooms are for one at a time. At least where I live. It strikes me odd that she would unlock the door while she was still washing her hands, but very odd that someone came in on her, and proceeded to do his business while she was still there, especially if it was an opposite-gender person.
There's plenty of gender-neutral bathrooms that have multiple stalls and 1 shared space where you can wash your hands. I'm assuming she meant she was in the handwashing space and not in a stall.

The gender neutral bathrooms I have used have almost always been one person only. I did use one once that had multiple stalls. There were no urinals. The stalls had doors that completely met the floor. So we all used the toilet in perfect privacy. I did not think about it until to my surprise I realized I was washing my hands at a sink next to a man. I don't think washing ones hands is very private.
Sweetheart, in a "gender neutral" bathroom the behavior of Man #1 was far more what you should have expected than that of Man #2. Man #1 treated you as an equal (gender neutral) and Man #2 acted like a stereotypical "gentleman" and treated you like a "lady." People like you really need to think about what you want. "Gender neutral" means just what it says: you should expect to see just about anyone in there.

I have only tried using a gender free bathroom once. I just felt really weird standing in line behind men and women I just decided to hold it in and wait until I got home hence it was an early night out that's for sure. Whilst I understand the philosophy about gender neutral bathrooms to me it's like losing just a bit more security. Maybe it's an age thing as I am in my 60's but I just refuse to use them when in a professional establishment.
What gets me about gendered restrooms is that it only matters if you're *using* the restrooms. As a female employee at a fast food joint, I was expected to clean the men's room. I asked that a male employee do that. I was told that all the male employees were cooks and I should just knock on the door first. I did and walked in on a elderly, hard of hearing gentleman in the middle of relieving himself. Thankfully, he was at the urinal right next to the sink, which blocked my view of anything below the belt. I have to imagine that was almost as uncomfortable for him as it was for me.
I've never seen a gemder neutral bathroom, but it seems like an awesome concept.
