Maybe Not The Most Chilling, But Frightening: American Airlines Flight 191
Mechanics had used a forklift to remove the engine and pylon as a single unit during a prior service, which cracked the wing's mounting pylon structure. They were supposed to take the engine off by itself. So when the plane took off, the engine fell from the wing. What's worse is that if the pilot had put full power to the plane, he might have saved it as these planes can fly with just one engine. But he throttled back, causing the plane to cartwheel and explode on impact. The seats inside had small television screens so the passengers could see themselves take off. Those were promptly uninstalled in all the other planes. There was a trailer park next to the airport and many of the inhabitants were retired police and firefighters. After the crash, they told of hearing people walk up to their trailer and then opening the doors and seeing no one, even after someone knocked. Or they saw a man walking away with his coat smoking. The people in question could be considered to be fairly reliable witnesses. People have also seen a man talking in an old phone booth inside the airport who then vanishes.
