One of the hearts has "Orthopedics" written in it in Cyrillic. So I think it was made for a doctor, maybe after their first amputation. Not for a patient after THEIR first amputation. But whoever made this cake must have studied ulcers really well, cause the Iodine ointment stain is exactly the right color.
