27 Years After Blink-182’s Enema Of The State, The Cover Nurse Looks Completely Unrecognizable
Few albums have a cover as immediately, viscerally recognizable as Blink-182’s Enema of the State. This 1999 record turned three idiots from San Diego into one of the biggest rock bands on the planet, and the image that came with it, a nurse in a red bra and a blue latex glove, became as much a part of the cultural furniture as the songs themselves.
Fifteen million copies sold. A generation of teenagers permanently imprinted. Travis Barker’s career launched into the stratosphere. And somewhere at the center of all of it, largely unrecognized behind the makeup and the uniform, was a real woman with a story that is considerably more interesting than most people ever thought to ask about.

But 27 years down the line, her life has taken a few colorful twists and turns
The woman in the nurse uniform is Janine Lindemulder, an adult film star who was already a well-established name in her industry by the time Blink-182 came calling. She had been a Penthouse Pet in 1987, had worked as an exotic dancer straight out of high school, and had built a career in adult films that earned her enough recognition to eventually land her in the AVN Hall of Fame in 2002.
The butterfly tattoos peeking out on the cover were hers, but the latex glove was the photographer’s idea, inspired by the album’s original title. Enema of the State was going to be called Turn Your Head and Cough. “Obviously, an enema is not really a glove type of thing. I thought it was a good visual,” David Goldman told the Huffington Post.

In the same year that Enema of the State dropped and proceeded to soundtrack an entire generation’s adolescence, Lindemulder announced she was leaving the adult film industry to become a kindergarten teacher. She wanted to focus on raising her son and pursue something entirely different. It is one of the more dramatic career pivots in living memory.
The retirement did not stick, and she returned to her X-rated roots in 2004 to make several more films, but the intention was genuine. By 2017, she had added an OnlyFans page to her portfolio, offering both free and VIP content at $10 a month, with a bio that lists her credentials in order: “Penthouse Pet ’87, Vivid girl, AVN Hall of Fame, Blink-182 cover model.”
But shortly after the release of the album, she tried to give up her X-rated career and become a kindergarten teacher
When Mark Hoppus, Tom DeLonge and Travis Barker picked her photo out of a casting pile, they apparently had no idea who she was. Producer Jerry Finn reportedly had to pull the band aside to explain that they had just hired one of the most famous adult film stars in the world.
Janine herself has spoken warmly about the experience, describing the band as being like little brothers. “The first time we met, they were very curious about the adult industry,” she told American Songwriter. “They’re like little boys, just curious.”
Her retirement didn’t last long, and she eventually made her way back to the industry with several more films and a stint on OnlyFans
Away from the album cover and the career changes, Lindemulder’s personal life was equally turbulent. She made an adult tape with her boyfriend, Mötley Crüe lead singer Vince Neil, and later married motorcycle customizer and TV personality Jesse James ( you might know him as Sandra Bullock’s cheating ex-husband) after roughly a year of dating. The relationship unraveled quickly.
By November 2003, while Lindemulder was pregnant with their daughter Sunny, the couple had already separated. What followed was an acrimonious split involving allegations of domestic violence from James and accusations of infidelity from Lindemulder. This was the kind of divorce proceedings that generate a paper trail nobody involved is particularly proud of.
She also married Jesse James, with whom she shares a daughter, and served time in prison for tax evasion
In August 2008, Lindemulder pleaded guilty to federal misdemeanor charges of willful failure to pay nearly $300,000 in federal income taxes, despite having made a down payment on a $647,000 house and purchased two new vehicles while fully aware of her outstanding tax debt. In December 2008, she began serving a six-month federal prison sentence, with the judge also ordering repayment of $294,000 in back taxes.
She eventually lost custody of the daughter she shared with Jesse James. James claimed her boyfriend at the time was also a convicted felon and that the environment wasn’t suitable for her. During the custody battle, Janine was arrested once more when Jesse filed harassment charges, claiming Janine phoned him more than 20 times a day. Fortunately, the whole affair is now behind her.

Now in her 50s, Lindemulder looks significantly different from the woman snapping a latex glove on the front of the most successful pop punk album ever made. Her hair is darker, and her tattoo collection has expanded considerably beyond the butterfly, but now she can add ‘hot grandma’ to her resume. Yes, you read that right; she is also a grandma.
She remains active on social media and continues to engage with the legacy of the cover, which, 27 years on, shows absolutely no signs of fading from the cultural conversation. But for once angsty teenagers wanting to live out a fantasy, she wrote on Instagram: “Being a grandma is awesome. This is why I don’t shoot anymore.”

If Janine Lindemulder isn’t the only album cover star who grew up. Spencer Elden, aka the Nirvana baby, became an overnight sensation in 1991, and he was only 4 months old. He had no say in the matter, no concept of what was happening, and absolutely no idea that his bare body would be reproduced on over 30 million album copies worldwide.
Now in his 30s, Spencer has had a complicated relationship with the image. He has recreated it multiple times as an adult, has spoken about the strange experience of being simultaneously famous and completely anonymous, and in 2021 filed a lawsuit against the surviving members of Nirvana claiming the image constituted child exploitation.
The lawsuit was ultimately dismissed, and the album cover remains in print. Spencer Elden and Janine Lindemulder will forever occupy a very specific corner of the cultural echelon, the people who, whether they planned it or not, shaped the visual memory of an entire generation of millennials.
