There are many things that can go wrong at work. But as far as your health and safety goes, your employer has to protect you and tell you about health and safety issues that may affect you. Workplace risk factors include but are not limited to chemicals, biological agents, physical factors, adverse ergonomic conditions, allergens, and psychosocial factors.
From making sure there are enough first-aid resources to minimizing potential risks at all costs, these are the least employees can do. And if not, the chances are they will get busted on the r/OSHA subreddit, which is entirely dedicated to reminding people to stay safe at work. Borrowing its name from the Occupational Safety and Health Administration, the community's description says “Safety First!” and its 745k members mean it.

My school was being completely remodeled my first year of high school. They were tearing down buildings and rebuilding one section of the school at a time. Construction crews hit a gas line causing a leak and everyone had to be sent home. As I was walking past the building with the leak, I saw a construction worker inside smoking a cigarette. I’m pretty sure he was fired and fined $500 for smoking on a school campus.

Oh my gosh, yesterday we were driving down the freeway and we saw what can only be described as some backwoods, redneck, stacking dolls. (we live in the city) There was a flat bed truck with a box truck on the back, in the box truck was a Toyota SUV and I swear to all that's holy inside that was a friggin washing machine. Like these people were moving and like, listen we're only doing one trip, so put the washer in the Toyota, put the Toyo in the box truck and throw that bad boy on the flat bed. Not surprised to see one from Malta. This kind of thing happens on most of the construction sites (of which there are MANY), and there are deadly accidents on a fairly regular basis. Last year a woman died in her own home because the construction site next door had weakened the structure of the building and her house collapsed on top of her.

Maybe a bit of something along those lines, but I suspect a lot of it is training, experience and exposure. I have fear of heights, even Uranophobia (term not used much, means fear of heavens, and if you feel like a giggle then there's an animation that explains this and other phobias graphically called Figaro Pho, can find it on Youtube) which can make it hard to even walk on solid ground let alone be at heights, but I've gotten it under control, have a working at heights and elevated work platform permit and can manage that, but still even watching those guys climb construction sites makes my balls quiver like my perineum is collapsing in on itself. So, I'd say, from personal experience, it's probably a bit of both.

I was working midnight shift as a nurse in a nursing home and saw white smoke rolling down from above the drop ceiling. I called the fire dept. and anticipated the possibility of evacuating residents. Thankfully it turned out to not be a big deal. Later I was told by the facility maintenance head that if it happened again that before calling the fire dept I should get a ladder and inspect the area myself to see what was wrong. I declined and said it wasn't in my job description. My husband the electrician wanted to strangle the idiot that told me to do that.
Doesn't seem too bad, depends on the size of the fire. Some fires are small enough that throwing a drink or stamping on it could put it out.
I think the O.P. read too much into these directions. Number 4 (Direct people from the area) is telling people to get away/out of the fire location/department. And Number 5 is somewhat ambiguous. It could mean that IF you CHOOSE to assist prior to the Fire Department arriving, do not be a hero going at it alone.

Those pliers can easily break loose and shoot out sideways. Not to mention what would happen if they came loose while installing the shock. Close but failed: the jaws of the hasp need to be in the lock--out hole. Here only one padlock is locking out, and the others are just ornamentally attached along with the hasp. Only the blue padlock is an approved Lock Out padlock the others are not.
Not sure what they are doing, but one way to secure something several people need access to is link the locks. Each person has their own lock and key. Remove one lock to open.
This is how it's supposed to be. In situations like this, every worker is required to put these safety locks in place for their own protection.
Yeah, there is no protection offered here. The lockout hasp needs to go through the switch. That silver bit that looks like a claw? It should be where the first padlock is. So one key and the whole thing comes away.

The "right equipment" was just inside those double doors. To me this indicates the the employee opted to go without. It's more likely in this scenario, that the worker felt comfortable doing the job his way and his supervisor/manager would have chewed his heinie if they became aware.
