Where there are machines and devices, there’s always a chance they may, one day, out of nowhere, just stop working. Sometimes, it’s just a mere nuisance, other times, if we're talking about big, major structures, the outcome may be catastrophic.

There’s even a whole corner of Reddit dedicated to the latter. The subreddit Catastrophic Failure is a place to share some unfortunate and peculiar scenarios of what happens when things like a chemical storage facility has a leak or when water lines freeze and burst in Texas during the record low temperatures.

So let’s scroll through the compilation below, which is honestly both frightening and a reminder that we all have to be more careful to make sure such accidents won't happen.

Me too. At the same time it's a weird psychological trick our mind plays on ourselves when one people dying feels worse than if the whole plane had crashed and everybody had died. I guess it's because we can empathize more easily for a single person or a small group of people than for a large number that becomes abstract.

I'm amazed at how many people survived. At that altitude, temperatures are freezing, and the air is thin. I'd guess a lot of people passed out before the pilot could get down to an altitude people can survive.

    A man was driving across the bridge at the time. He had his daughter's cocker spaniel with him. The bridge deck was swaying and twisting so badly he had to crawl on his hands and knees off the bridge. The little dog was too frightened to get out of the car. So I guess there was a casualty that day. Bridge was rebuilt and a few years ago another bridge was built along side it for southbound traffic (I think for southbound). Check out the video.

    Improper? I know where that building is and I saw it everyday. It had a pit inside a 'factory' and the owner filled it with this stuff and it leached for over a decade. The Chromium was found in a local river. He deserves a lot of years for what he did and the local official I believe knew about it too.

    We had an interesting case of industrial pollution in the Denver, CO area in the late 1970s into the 1980s. Manhole covers kept blowing off in a small neighborhood. It did not take long to track it back to a corner that had 3 gas stations. It took a few years of research and a trial to get the offending station to clean up their mess. One of their underground storage tanks had sprung a leak and they were releasing gasoline into the groundwater. They drained the tanks and removed them. They dug up all of the affected soil and hauled it to a hazardous waste dump. There were several (10 ish) homes that had detectable gasoline in their basements. The Oil company had to buy all of the homes at market value. Only three of the homes could be cleaned up enough to sell. The other homes were demolished and the land became a park. If not for fortune, allof those homes could have blown up! It was shortly after this that EPA issued the regulations on underground tanks and the monitoring requirement

    The fault was probably not at the battery directly, but in a wiring harness or within the power distribution panel itself.

    I learned earlier this year that it wasn't an instantaneous death, either. They survived for several minutes while the piece they were in descended back to earth. That has got to be a horrifying way to go. I'd rather just have exploded.

    Commercial planes are all required (and tested) to be able to take off, fly and land with 50% engine capacity, FYI. Don't panic if it happens to you!

    If you're afraid of flying, you should know that everything on aircraft is built redundantly so that if one system fails the other can take over. What that means is that one engine down is not enough to take down a plane (usually) which is probably why the crew was able to land the plane safely. Also you would have to fly non stop for 2000 YEARS to risk dying in a plane crash. A very low probability. Hope that helps. Be safe.

    The ARFF rigs from my firehouse were the first ones on scene when it landed. We put it out on the runway using just two rigs. When this happened it left parts of the plane in several areas of the suburbs north of Denver.

    That’s close to my hometown. I pass it every day. The structure itself was saved, because the burning reed fell off, so it didn’t burn much, and they rebuild it. In fact, they just readded the top last wee and are almost done now.

    Interestingly the reason why they were drilling there is the safer ocean areas to drill they were denied access to by the US govt who gave them permits for this spot. Then the US govt gave them exemptions to certain safety standards because it was the only way to get the tax revenue because BP said they wouldnt drill otherwise. On top of that, Inspectors were assigned to specific companies and were allowed to take gifts, which led to many of them being bribed as well. This was just as much the fault of government as it was BP

    Just today i listened on the radio about current situation. Some of the politicians, who were in power during the explosion, got elected into parliament. Until they are sworn in, they do not have immunity. Judge tried to used this to call former ministers to testify. So one of them filed a complaint against the judge. While this complaint is being investigated, the case of the explosion cannot proceed.

    Speaking of catastrophes themselves, there have been far too many of them in the past and recent history. Since one of them, the Aloha Airlines flight 243 tragedy in the skies, over 30 years have already passed. The incident happened when cruising at 24,000 feet: an 18-foot section of the plane's roof suddenly ripped off. It caused an explosive decompression, creating a gaping hole in the fuselage and sucking a flight attendant out of the plane.

    After the incident, a passenger reported that before boarding, she noticed a crack in the fuselage, but she did not report it prior to takeoff. Later, a National Transportation and Safety Board investigation suggested that the incident may have been caused by the plane's old age and poor maintenance. Moreover, Aloha Airlines short flights like this should have received check-ups twice as often as they had.

    I saw a video where a tire blew out, took out the one next to it, then they caught fire. Pilots did not see this, so they retracted the gear like normal. The tires, which were still on fire, then burned through the cabin. The pilot tried to land but missed by 1 1/2 miles. 261 dead.

    Another peculiar incident happened a year ago at a Swiss chocolate factory where, due to internal malfunction, it sent out a plume of cocoa 'snow.' Fine particles dusted the ground around Lindt & Spruengli factory in town of Olten after the ventilation system stopped functioning properly.

    The area where the malfunction happened was in charge of making roasted cocoa nibs. These nibs are fragments of crushed cocoa beans, which are the basis of chocolate. Due to the strong winds, the powder spread around the factory, covering it in fine cocoa dust.

    A year ago, a giant whale sculpture became a national hero in The Netherlands for stopping a train that overran the tracks. Luckily, the driver of the train in Rotterdam city escaped serious injury as the train crashed through barriers at the final station of De Akkers in Spijkenisse, near the Dutch port city of Rotterdam. The accident occurred on a raised platform, ten meters (30 feet) off the ground, where the carriage could have easily plummeted into the water below.

    Maarten Struijs, the architect who designed the whale sculpture, expressed his astonishment and relief that the structure had played a life-saving role. “I'm surprised it's so strong," he told a Dutch news outlet. "If plastic has been standing for 20 years, you don't expect it to hold a metro carriage."

    Despite the rich people's d*** length contest it can actually help the planet. Satellites can be used for reducing fertilizer useage by showing areas where they are necessary and where not. Or the state of forests. The GPS system benefits a lot of emergency services and enviromental causes, too.

    It's a Proton. It has no SRBs. What you see there looking like the clustered SRBs of a Delta are actually the stage one liquid fueled engines clustered around the stage one tank in the middle. Also, it uses hypergolic fuels, namely Unsymmetrical Dimethylhydrazine and Nitrogen tetroxide, which are a lot more toxic and dangerous than mere SRBs. Oh, and since they're Russians, they don't use any flight termination system, they just let the rocket impact the ground, since they launch from such remote areas. In this case, the rocket disintegrated due to aerodynamic forces before it impacted.

    I just think about all the older workers who instantly volunteered to go in and control the complete overload. Knowing that they would quickly die from radiation poisoning. They determined that they had lived more of their lives, and they should do whatever necessary to protect the younger workers and all the millions of people that would be affected by a full scale nuclear meltdown. These men were just ordinary blokes, everyday workers. Nobody ever mentions their heroism or sacrifice. They all died.

Fukushima, the meltdown directly resulted in the deaths of two people. 2014 report by the United Nations Scientific Committee on the Effects of Atomic Radiation and World Health Organization projected no increase in miscarriages, stillbirths or physical and mental disorders in babies born after the accident at Fukushima.     It was explained to me that grain storage silos are built with "light" materials to reduce the chances of huge "shrapnel" explosions when grain silos blow up. Silos, if built out of "harder" materials will, generally, blow UP, like a cannon shooting into the air. Thus a huge dispersal of shrapnel. Explosions in metal silos will collapse the silo. Or at least that's what an engineer told me.

    Going to guess the bomber wasn't carrying bombs and was probably not full up on fuel, unlike the 767 which had just taken off and WAS full of fuel. Not sure, this is the first I've heard of the incident.

    Actually if you do just a quick fact check this is a wall that was currently under construction (see the workers) and not during hurricane Hanna but a high wind event it wasn't structurally sound bc it was still under construction

    It's cost cutting over the last few decades not just now, it's just that the hens are finally coming home to roost. The US has some very bad stats on the safety of their bridges but they're not the only country by far. The most recent big one was in Italy and for the same reason - basically hadn't been regularly maintained over decades. I think they were actually fixing that one at the time it collapsed, as they may have been in Minneapolis too, but if your bridge is on the verge of collapse and you add heavy construction vehicles and vibrating equipment to the equation without also closing it and adding extra support, boom, no more bridge.

    This happened in October of 2019. They were finally able to get the bodies of 2 of the men who died out in August of 2020, almost a year later. And there was no way the hotel was due to be open in a month, they didn't even have the outside cladding on the building. This happened because of corrupt contractors & city inspectors colluding to use substandard materials & shortcutting standard construction methods.

    You are supposed to be ready to take over immediately but the idea that you can trust humans to be in that ready state permanently when they already lose concentration on cars that require full time seems a bit unrealistic. Still probably be safer ultimately as speed, distance from other cars will all be automatic (when we are all driving them). Though if the systems go down (as they do) brown trousers time.

    The problem is a very small amount of fire crews are equipped with the necessary tools to put out an electrical fire, for obvious reasons you can’t just use the hose and spray water on it. If you get trapped in an electric car fire the your going to die same accident in a standard car there’s a chance you could be saved

    Looks like steel production. The red hot steel runs along machiery to "string it out" for bringing it into shape for rods, bars or similar. If it loses track and jumps out of the bearings it will go anywhere due to production speed.

    Known as a cobble, this is a not-uncommon occurrence in hot mill fabrication. There are some pretty cool videos on Youtube. Search for steel mill cobble. It's fun. But I don't know how it got up to the ceiling.

    The piston was not in the engine itself. It's called a ratio boost piston. Its part of the fuel system that feeds the engine. Because of the malfunction, one engine was not getting enough fuel and was at idle speed. Because of the misbalance between the two engines, it caused the aeroplane to become uncontrollable.

    Of course they do, they make a really nice bright light when mixed (i think I had a chemistry school book with a picture of a diver carrying a magnesium torch, so it can it really can be usefull if you know what you are doing. But yeah, doesn't look like they did their chemistry)

    They are called "bleve". A pressure vessel is fine when it's full and on fire, it's when it is empty with no pressure keeping the fire outside of the vessel that there is a problem

    This appears to be a "double width" modular structure, they are usually moved one half at a time. I wonder what compelled them to move the thing while assembled.

    If I remember correctly, this test was designed to basically pump up the tank until it exploded to see what it could handle. No catastrophic failure here, just a really, really extreme stress test
