But many refuse to pay local staff appropriately so they have almost 0 overheads, and this is the result.

No, this is absolutely not true and that's exactly the fallacy I'm talking about - what you may think is common sense and universal standard is in fact trained and cultural. Not everyone thinks of mold as a death threat! This "universal concept" assumption is why tourists constantly get sick from street food. Too many people imagine that things in developing countries are just as good as what we have, but for a fraction of the cost. It's a fairy tale. Cleanliness, education maintenance and elevated nationwide standards all cost money. Of course you can still travel and be safe for cheap, but you have to be vigilant and expect some risks and sacrifices - very basic example, it will be common to go right from the bathroom to food prep without washing hands or applying gloves.
